Our large gaming community also delivers stability and a democratic structure few guilds can rival. We hold Division and Community elections every six months, keeping leadership fresh and avoiding burnout. A detailed Charter enables good governance and financial stability for an organization designed for the long term. A strict but fair disciplinary system ensures that any toxic behavior is addressed quickly and confidentially.
This guild structure allows a safe and joyful space for players of all backgrounds. Many of our leaders and members are part of the LGBTQ+ community, and we have players in every generation from Zoomer to Boomer. Our commitment to antiracism, diversity, and inclusion is built into our foundation and implemented through all our policies, including accommodations as appropriate.
The larger gaming community additionally provides many opportunities outside GW2. Our Discord and forums are constantly active, and the community meets as a whole once a month. We have a range of cross-community activities, including karaoke, movie nights, multi-game fashion and other shows, and casual games. I’m not a native english speaker so sorry in advance if this post contains any grammatical errors. I’m neither a doctoresearcher or a science guy so I’ll try to alway provide some reference but be aware that the information that follows may be misinterpreted or simply not correct.
The aim of this post is to understand what is the consensus and the consolidated evidence on PE, and what information we should expect from a clinician/doctor (so you probably won’t find any new therapy you have never heard of). I’m curious and wanted to know a little bit more about this topic since I think there is a lot of misinformation and common beliefs that are rooted but are not backed up by any scientific evidence and even doctors/clinician sometimes are not educated enough on the topic to provide any actual help.Don’t expect anything super well summarised, this is going to be more of a copy and paste work.
Following the Hierarchy of Evidence in medical research meta-analysis, systematic reviews and practice guidelines provide the highest quality of evidence, I focused on the guidelines because I think that are more practical and have more reach to doctors/clinician that are not expert in the field. I found the ISSM’s (International Society of Sexual Medicine) guidelines to be the most complete guidelines on the topic. The International Society for Sexual Medicine (ISSM) is a medical society devoted to the study of the medicine of human sexuality. It publishes two journals, The Journal of Sexual Medicine (where the guidelines were published, IF for 2022-2023 is 3.9), and the open-access Sexual Medicine Reviews. The last update of the guidelines I found is from 2013/2014, American Urology Association has its own guidelines updated in 2020, they are not specifically focused on just PE but on Disorders of Ejaculation, mainly PE and DE (Delayed Ejaculation). The AUA’s guidelines focus mainly on the management of the patient and the pharmacotherapy and don’t give many indications about the main theories on PE’s causes.

Definition from ISSM

The panel proposed a unified definition of LPE (Long Term PE) and APE (Acquired PE):
It “is a male sexual dysfunction characterized by:
  • ejaculation that always or nearly always occurs prior to or within about 1 minute of vaginal penetration from the first sexual experience (LPE), OR a clinically significant reduction in latency time, often to about 3 minutes or less (acquired premature ejaculation);
  • the inability to delay ejaculation on all or nearly all vaginal penetrations; and
  • negative personal consequences, such as distress, bother, frustration, and/or the avoidance of sexual intimacy.”
It also identify additional subtypes: VPE (Variable PE) that occurs irregularly and inconsistently and SPE (Subjective PE) a subjective perception of PE but with the IELT in the “normal range” (so a IELT >= 3 mins, the median IELTs from 2 studies is 5.4 minutes and 6 minutes respectively).
The estimated prevalence of APE and LPE is around 5%, the prevalence of VPE and SPE is higher but these definitions are relatively new so there isn’t a lot of data.

Etiology (form ISSM’s guidelines)

I start by saying that of all the biological factors that have been proposed to explain PE, none has been confirmed in large scale studies, nonetheless some studies are really interesting.
Hypersensitivity of the Glans -> in a small study was found that patient with PE have hypersensitivity and hyperexcitability of the glans via performing somatosensory evoked potentials (brain responses to sensory stimuli). A similar study not cited in ISSM’s Guidelines is Zhang 2009 that found in a small cohort that patients with PE have on average a higher number of dorsal penile nerve branches. Disturbances in central serotonergic neurotransmission -> We know that Serotonin is the main neurotransmitter involved in the control of ejaculation, but in humans specific subtypes of 5-HT (serotonin) receptors involved in PE have not yet been identified. SSRIs work but SSRIs treatment activates many different postsynaptic subtype receptors so we don’t know specifically how it works. In the spinal cord, specific neurons have been found to be essential to ejaculatory reflex in rats and there is also preliminary evidence that such a neural organisation also exists in humans. Genetics of PE -> Some studies have investigated gene polymorphism of receptors of serotonin and oxytocin. Results of the studies are mixed, men with some genetic variants may be predisposed to develop PE but more studies are needed. Hormones imbalances -> Hyperthyroidism has no role in LPE and has been found to be associated with APW in extremely few patients. Recent studies on large population indicate that the endocrine system is involved in the control of ejaculatory function: prolactin in the lowest quartile levels is associated with APE and anxiety symptoms and higher testosterone levels aìcorrelate with PE, while lower androgenization is related to delayed ejaculation. Anyway the guidelines state that both low PRL and relatively high testosterone levels cannot be considered etiologies of PE. Prostatitis -> Prostatic inflammation and chronic bacterial prostatitis have been reported as common findings in men with APE. For the guidelines though there is insufficient evidence to support routine screening of men with PE for this condition. Erectile dysfunction -> For the guidelines ED is unlikely as a comorbidity or etiological factor for LPE, there is data to support that APE is associated with PE. Psychological factors -> there has been limited research on causality; most studies can only report association. It is plausible that psychological factors may lead to PE or vice versa, it is likely that for many men the relationship is reciprocal with either PE or the other factor causing exacerbation of the other.

Psychological/Behavioral interventions

There is not much to say on this topic, well designed studies on behavioural interventions are scarce, apart from the squeeze/start-stop technique other interventions are correct breathing and relaxation of the muscles to control excitement (discussed a lot in this community).

Pharmacological Treatment (from ISSM guidelines)

Dapoxetine: it is a rapid acting and short half-life SSRI with a pharmacokinetic profile supporting a role as an on demand treatment for PE. In randomised controlled trials, dapoxetine 30mg or 60mg taken 1-2 hours before intercourse is more effective than placebo from the first dose resulting in a 2.5 and 3.0 fold increase in IELT. Treatment-related side effects were uncommon, dose dependent and included nausea, diarrhoea, headache and dizziness: they were responsible for study discontinuation in 4% (30mg) and 10% (60mg) of subjects.
Off-Label SSRIs and TCAs: Daily treatment with off-label paroxetine 10-40 mg, clomipramine 12.5-50 mg, sertraline 50-200 mg, fluoxetine 20.40 mg and citalopram 20.40 mg is usually effective in delaying ejaculation. A meta-analysis of published data suggests that paroxetine exerts the strongest ejaculation delay, increasing IELT approximately 8.8-fold over baseline. SSRIs should be avoided in men with a history of bipolar depression.
Topical Local Anesthetics: lidocaine and/or prilocaine as a cream, gel, or spray is well established and is moderately effective in delaying ejaculation, data suggest that diminishing the glans sensitivity may inhibit the spinal reflex that are responsible for ejaculation. PSD502 (Fortacin in Europe) , a lidocaine-prilocaine spray applied 5 minutes before intercourse is associated with a 6.3-fold increase in IELT. There were minimal reports of penile hypoanesthesia and transfer to the partner due to the unique formulation of the compound.
PDE5i: SIldenafil, Tadalafil, Vardenafil, are effective treatments for ED, there is some evidence to support the efficacy and safety of off-label on demand or daily dosing of PDE5i in the treatment of LPE in men with normal erectile function but further evidence based research is encouraged to understand conflicting data.

Other pharmacological treatments

Tramadol: Tramadol has been investigated as a potential off.label therapy for PE, with several studies demonstrating efficacy, the mechanism of action is not completely understood. It may be considered when other therapies have failed because of the risk of addiction and side effects.
Oxytocin: Intraventricular administration of oxytocin antagonist inhibits sexual behaviour in animal studies, however in one human study, an oxytocin antagonist failed to clinically or statistically improve IELT, further human studies are necessary.
Alpha1-adrenoceptor antagonists: not mentioned in ISSM’s guidelines but in the AUA’s. There are not good quality studies on these drugs, Silodosin seems the most effective one. From AUA

Other treatments

Cryoablation and Neuromodulation of the Dorsal Penile Nerve: invasive and irreversible procedure, which is associated with an increase in the IELT. However, safety of this treatment modality needs to be determined before this procedure can be recommended for treating patients.
Intracavernosal Injection for PE: There is limited evidence regarding the efficacy of intracavernosal vasoactive drug injection for the treatment of PE.
Injections with hyaluronic acid in the glans: a study cited by AUA found a 2.6 fold increase vs 1.1 placebo in ELT, AUA recommends that surgical intervention for PE only to be considered in the context of ethical board approved trial for patients who have failed or cannot tolerate alternative management strategies for PE.

Preclinical studies for PE

Several agents have been studied in animal models for treating PE: a potent SSRI (DA-8031) significantly inhibited ejaculation in rat models. There are some human studies after 2013 (so after the publication of the guidelines) but I can’t understand in which phase the drug is and if the possible therapy with this SSRI is on-demand or not and the drug is not mentioned in AUA’s guidelines and I don’t understand why.
Modafinil, used in the treatment of narcolepsy, produced significant delay in ejaculation in rat models. Can’t find a lot of studies after 2013, in the AUA’s guidelines form 2020 it’s reported that in an uncontrolled pilot study of on-demand modafinil it’s reported a modest but significant two-fold increase in IELT.
Injection of botulinum toxin in the bulbospongiosus muscle, from AUA’s guidelines: the toxin that blocks neural transmission when injected into the muscle. But studies for PE were discontinued due to lack of efficacy in interim analysis.
Oxytocin antagonists (from AUA’s guidelines): Cligosoban is a oxytocin receptor antagonist that in a RCT double blind trial showed statistically significant effect of LPE men (3.6 fold increase in ELT vs 1.8 for placebo).
Further study and potentially development of agents with different distribution is required to determine if these therapies will have a role in PE management in the future.

There are only two type of people in this game.
  1. People who are better than you. These are the people who played the game since it came out. They know every stam break, every meta, and know every combo off muscle memory. They don't let you charge, use ki stuns and counter limit burst most of the time. These people also know when to perfect block efficiently, unlike you block-spamming gremlins out there. Damn dry-lip mouth-breathers..
  2. People who are worse than you. This consists of the cheesers and the people who just got the game and immediately hopped into PvP without finishing the game or reaching Level 9. These people also use the annoying moves that have Super Armor or i-Frames, always stall until the clock runs out, and found one dumb strategy and stuck to it, making people like you and me especially mad because we know we can beat them if only the game had some kind of netcode update.
There is NEVER an in between. Every match I've never played was either straight wins or straight losses. I hope you agree lol.

2023.06.04 17:43 JustCuriousArizona So, Big Deal, the network is radically changing, then Why should I invest in Nokia, when and how?

So one of the things is you may say fine, "The network as we know it today is changing radically for tomorrow?", I'll accept that, but "Big deal, what does that mean to me as an investor, i.e., why should I invest now into Nokia, when and how and what is my expected YOY reasonable return?
Following are items you need to recognize if you invest in Nokia:
  1. Margins are low, unlike Cisco they do not have substantial service and software business, this will take years to develop
  2. CSP's YOY are likely to implement the "new network" at a depreciation rate of the old 5G network, which is likely a 10% depreciation schedule. What this means is if the total installment base presently is $500b USA dollars, then we are looking at a total "new network" of $50b (if you have better numbers here please let me know and I will modify this statement, I am just guessing here, based upon present 5G TAM which is probably mostly depreciation replacement now).
  3. New business ventures likely to have a break even 3 to 5 years:
    1. SaaS, Private Wireless, Services
    2. Breakeven likely to be 2026 or 2028, meaning margins will increase after that year.
  4. Should see a steady rise of absolute revenue and earnings, note do next expect margins to go up.
    1. Any drop in revenue will be seen as severe, that Nokia business plan isn't working, the CEO will have to explain the drop, if the drop is due to the market place slowing down then the drop will be around the same as the market. If the drop is due to the fact that Nokia's business plan isn't working or is slower than projected then Nokia stock will drop a lot more.
  5. As the new network becomes a reality to the market place, the PE and P/B ratios should go up.
    1. Target PE = 10, P/B=2 in 5 years, this is market valuation of the company to the market place.
    2. Given the above, expected CAGR rate for Nokia in a 5 year window would be about 15%
      1. Note the above will be very non-linear, most of the growth is likely to happen in the last 2 years of a 5 year window.
      2. Within the 5 year window, the 1st 3 years probably a 2 to 5% CAGR growth rate in Nokia stock price
      3. Nokia's trading range for the next 5 years is likely to be in the +/- 15% to +/- 25% range about a mean price.
    3. Will only go up if the QOQ shows competence reports build up a case that Nokia's business plan is working, meaning it has to hit the revenue and earnings estimates.
The above outlines the reward and risks of buying into Nokia.
My opinion of how one should invest into Nokia, below is a conservative investment strategy:
  1. Do Dollar Cost Averaging, DCA, over the next 5 years.
  2. Focus on the monthly trading mean of the stock, measured over a two year window (at least) maybe 3 years.
  3. Realize that Nokia stock is likely to range about the mean by +/- 15% to +/- 25%.
    1. Double down when at the bottom of the range if you believe the company is still on course, present Q1' 23 is a great example, Nokia is still on course, IMO one should double down.
      1. Signs to double down by are Pekka doesn't change his year end targets, no major changes in Nokia's business plan, revenue increases.
    2. Do not buy anymore if it looks like the wheels are coming off of Nokia business plan.
      1. Also if the wheels start coming off, consider doing your DCA over two companies. What I mean is that presently Nokia IMO looks better than Ericsson, so I wouldn't buy Ericsson, however if Nokia starts doing bad, then consider buying Ericsson, most likely if Nokia is starting to look bad long term it will be Ericsson who will be benefiting.
      2. The other thing to do is split your DCA over two companies looking good right now, which would be Cisco and Nok and keep Ericsson in your pocket to start doing DCA if the wheels start coming off either Nok or Cisco.
  4. If the market should crash
    1. Make sure Nokia has plenty of cash, Nokia is a bit light on it's cash position given the market metrics but isn't too bad. Lightness is measured by X months where cash will have to be used to pay dividends to the investor and to support R&D, Production as well as SG&A due to revenue drop of X months before recovery. Nokia does have more cash than Ericsson, which is good and important, but I would like to have Nokia have more like 7 to 8b USA dollars in cash instead of $5b.
    2. Make sure debt/equity is below 0.25, Nokia's is 0.17 (BTW debt to equity will vary by industry, for example debt/equity for CSG's probably can be as high as 2.0, but lower the better).
    3. Buy on the monthly when the present month candle breaks the previous month candle or to be super cautious when the present month candle breaks 1.5x high. Also do NOT be surprised if the stock goes down again, this is a start buying or start your DCA again. Do this only if you think Nokia is going to survive, given it's present books Nokia is very likely to survive a market crash.
  5. Evaluate your return over a 5 year window, NOT day to day (DTD).

Learning to save money, especially finding ways to do it quickly, is something that can benefit anyone. There are many good reasons to start saving money. Maybe you suddenly got slammed with an unexpected bill, or maybe your friends just invited you on that trip of a lifetime. Or perhaps, you want to buy a house and you have to save for the down payment. You could even just be trying to grow your emergency fund so that you are better prepared for any unexpected expenses. Whatever your reason may be, there are a lot of ways to save money!
Number 1 - Track Your Spending And Create A Budget Tracking your spending is the very best way to identify areas where you can save money. All you need to do is track your spending for one month, and this will give you a good idea of where your money is going. Once you’ve identified where you’re spending your money, and you see areas where you can reduce spending, you can set a reasonable budget and then stick to it. This is a simple thing to do, and it’s a very effective way of controlling your spending.
Number 2 - Pay Off Your Debt If you’re carrying a credit card debt, you’re wasting money. As difficult as it may be, your top priority should be to pay off your debt and free yourself from those high interest rates. Hopefully, some of the other tips on this list will help you retain more of your cash that you can then use toward conquering your debts.
Number 3 - Automate Savings Transfers Each Payday If you can’t seem to create a habit of saving, scheduling automatic transfers to your savings account can be a huge help. When you have a certain amount of your paycheck automatically transferred to your savings account each payday, there's less temptation to spend it - and you can easily watch your account balance grow over time. Review your budget and choose an amount that you can commit to regularly, then put your savings on autopilot.
Number 4 - Negotiate Your Bills While some of your bills, such as your rent or mortgage payment, may be non-negotiable, you may have some wiggle room with others. For example, you may be able to find a better deal on your car insurance or your cell phone service. Doing some research to find better rates, can take a little bit a time - but it can be time well spent, if it helps you save money.
Number 5 - Set Up Automatic Payments For Bills With our busy lives and busy schedules, it’s not uncommon to forget to pay some bills on time. An easy way to save money, is to simply pay your bills when they’re due. Companies typically charge a late fee for any balances that are overdue. And while it may just be a few bucks here there, these fees quickly add up - especially if you pay multiple bills late. So, set up automatic payments for bills to ensure that they’re paid on time, and to avoid any late fees. It’s also important to keep an eye on your bank account balance to avoid overdrafts and accumulating additional fees.
Number 6 - Go Cash Only Put your credit cards away, and then take out a limited amount of money from your checking account -enough to last you for a few weeks. Basically, you withdraw a limited amount and then watch it shrink. Since we're more motivated by loss than by gain, each dollar you physically spend will force you to spend consciously.
Number 7 - Consider Relocating Perhaps you live in the downtown core, and your mortgage or rent costs you 2-3 times more than it would if you moved just 15-20 minutes outside of the area. Relocating to an area with a lower cost of living or downsizing your home, could potentially put hundreds to thousands of dollars in your pocket each month. Obviously there can be some roadblocks that may prevent you from moving, but if relocating is an option, it may be well worth considering.
Number 8 - Stop Paying For Convenience It’s the American way to pay for convenience. People are willing to pay $5 for a taco they can make at home for less than $1. They pay $6 for a cup of coffee at a local cafe rather than brew an entire pot of coffee at home for a few pennies. Taking a little extra time out of your day to make your own food, brew your own coffee or clean and repair things around the house can grow your bank account rather quickly.
Number 9 - Make A Grocery List Making a grocery list before you head out, will quickly save you tons of money. This will ensure that you end up buying only what you need, and that you don't fall victim to any impulse purchases. Write down everything you need for the week. The less times you go shopping, the less likely you'll be to pick up something you really don't need. Plan to shop for an hour or less, and try to race the clock when you shop. This way you won’t spend time wandering around picking up things that look appealing. Also, plan to go shopping shortly after you've eaten. Everything will look less appealing if you're shopping on a full stomach.
Number 10 - Downgrade Your Cable, Phone And Internet For most families, these three services equal big bucks every month. Monitor your use over a month or two, and decide what you actually need and what you could cut. Do you really watch any premium channels? Is your landline phone doing anything other than collecting dust? How fast do you need the internet to be if you’re only checking Facebook and email? It truly pays to shop around and find a cheaper service.
Number 11 - Cancel Paid Subscriptions, Memberships And Services Are you subscribed to a magazine that you never read? Are you paying for a product delivery service that you hardly ever use? If you have a gym membership, when was the last time you actually made an appearance? Paid services, subscriptions and memberships can really add up. Make a list of all the ones you have, and ask yourself if you really need them. If the answer is NO, it’s probably time to cancel.
Number 12 - Quit Your Bad Habits For most people, it’s not easy to quit smoking, drinking, using drugs, or overeating - but these habits are costing you more than just the price of your vice. Quitting destructive habits will improve your health, lower your insurance premiums, and save you a surprising amount of money.
Number 13 - Buy Something New When You Replace Something Old If you tend to buy things only because they’re on sale, or just because - perhaps it’s time to stop because you’re wasting money. By establishing a rule that you can only buy to replace something you already have, you're creating an 'active barrier.' Before buying anything, think about how many of those you need, and how many you already have! Then think again, if you really need a new one. The psychology of having to open up your closet, decide what to give away, and get it to the nearest charity (or garbage can) is enough to stop many of us from buying something new.
Number 14 - Practice The 30 Day Rule The 30-day rule is a simple method to control impulse spending. Here's how it works: Whenever you feel the urge to splurge - whether it's for new shoes, a new phone, or a new car - force yourself to stop. If you're already holding the item, put it back. Leave the store. When you get home, take a piece of paper and write down the item, the store where you found it, and the price. Also write down the date. Now post this note somewhere obvious: a calendar, the fridge, or a bulletin board. For the next thirty days, think whether you really want and need the item. If, after the 30 days, the urge is still there, then consider purchasing it. That's all there is to it, but it's surprisingly effective. The 30-day rule works especially well, because you aren't actually denying yourself - you're simply delaying gratification. This rule has another advantage: it gives you time to research the item.
Number 15 - Take The Time To Comparison Shop This tip goes hand in hand with the previous point. The retail industry thrives on impulse, luring you with so-called sales that urge you to make an immediate purchase. Although there are some door buster deals that may actually be worth it, more often than not, you’re better off taking your time and comparing prices with other retailers. Among the many things you should consider, are not only the base price of an item, but also any shipping costs, coupon codes, and other offers. As an added bonus, during the course of your comparison shopping, you may even realize that you don’t actually need the item you’re looking to buy!
Number 16 - Watch Out For Fear Of Missing Out Your favorite social media site may be super addictive and offer plenty of useful advice. But they can also lead to fear of missing out. You’ve probably seen dozens of articles - that whether intentional or not, make you feel guilty about what you’re not doing. Such as: • Things you should do in your 20s or 30s • At what age you should buy a house or car • What luxury items you need to own … and so on Ask yourself, are they really things YOU want to do or buy? Or are you checking off items on someone else’s “bucket list”? Create your own list of goals, focus on them, and let go of the rest.
Number 17 - Turn Trash Into Cash Another way to bring in more money is by selling things you no longer need. Look for designer items you don't wear, electronics you aren't using, old books, or anything else you can put up for sale on eBay or Craigslist. Figure out what your stuff is worth - so you get a fair price - and stay safe by following best practices, like meeting buyers in a public place.
Number 18 - Earn More Money Using A Skill You Already Have Most people only think about cutting costs. This often leads to reading silly articles online that seem to only suggest ridiculous tips on how to be frugal. We forget about the possibilities of earning more money - which is the most powerful of all. Try negotiating your salary at work, starting a second job, or freelancing in something you're really good at. With this extra income, you’ll be surprised how fast your savings account will grow. Saving money is not as hard as you think.
You can save a significant amount of cash just by making small changes. And the best part is, not only will you learn to value money, but in the process of saving, you’ll also learn which strategies work best for you - so you can use them again when needed. Or maybe you’ll keep saving - that way you’ll always have some cash handy to cover whatever expenses come your way.
